Advance Health Care Directive
An "advance health care directive (ACHD)" or simply "advance directive" may refer to either (possibly both) a 'power of attorney for health care (PAHC)' or an 'individual health care instruction'. These documents are extremely important and are used to define your wishes concerning health care issues should you become incapacitated and are unable to make decisions or explain them to others.
A PAHC is used to grant another person (called an 'agent') the power to make health care decisions for you. These decisions may include: whether to approve or refuse a recommended diagnostic test, surgery or program of medication; whether CPR should be used and whether or not to provide, withhold or withdraw food and water. The PAHC may also grant the agent power to make personal care decisions for you. These include matters beyond health care such as where you will live and arranging for your recreation and transportation. Unless you restrict your agent's power under the PAHC, he or she will also be able to authorize an autopsy, make anatomical gifts and direct the disposition of your remains.
